Brilinta (ticagrelor) is a potent oral antiplatelet medication prescribed to reduce the risk of serious cardiovascular events including heart attack, stroke, and cardiovascular death in patients with acute coronary syndrome (ACS) or a history of myocardial infarction. Precautions

Contraindications

Do not use Brilinta if you have a known allergy to ticagrelor or any component of the tablet. Brilinta is strictly contraindicated in patients with active pathological bleeding (such as peptic ulcer or intracranial hemorrhage), a history of intracranial hemorrhage, or severe hepatic impairment, as the drug is extensively metabolized by the liver. Co-administration with strong CYP3A4 inhibitors such as ketoconazole, itraconazole, or clarithromycin is also contraindicated due to significantly increased ticagrelor exposure.

Possible side effect

Common side effects of Brilinta ticagrelor include dyspnea (shortness of breath), headache, dizziness, nausea, and minor bleeding such as bruising, nosebleeds, or gum bleeding. Dyspnea is a well-characterized effect occurring in approximately 14% of patients and is usually mild, transient, and does not require treatment discontinuation. Serious adverse effects requiring immediate medical attention include major bleeding events (gastrointestinal hemorrhage, intracranial bleeding), severe allergic reactions (anaphylaxis), ventricular pauses or bradyarrhythmias, angioedema, and elevated serum creatinine or uric acid levels. Any unexplained or prolonged bleeding should be reported to your healthcare provider immediately.

Drug interaction

Brilinta may interact with medications that increase bleeding risk, including anticoagulants (warfarin, heparin, direct oral anticoagulants), NSAIDs (ibuprofen, naproxen), aspirin above 100 mg daily, SSRIs, and other antiplatelet agents. Strong CYP3A4 inhibitors (ketoconazole, ritonavir) can significantly increase ticagrelor levels, while strong CYP3A4 inducers (rifampin, phenytoin, carbamazepine) can reduce its effectiveness. Simvastatin and lovastatin doses exceeding 40 mg should be avoided during Brilinta therapy. Inform your doctor about all prescription drugs, over-the-counter products, herbal remedies, and supplements you are currently taking.

Missed dose

If you miss a dose of Brilinta, take it as soon as you remember unless it is almost time for the next scheduled dose. In that case, skip the missed dose and take the next dose at the regular time. Do not double the dose to compensate. Maintaining consistent twice-daily dosing is important for optimal antiplatelet protection.

Overdose

Brilinta overdose may significantly increase the risk of severe or life-threatening bleeding and may cause pronounced dizziness, hypotension, or ventricular pauses. There is no known specific antidote for ticagrelor overdose, and the drug is not expected to be dialyzable. Seek immediate emergency medical attention if overdose is suspected and provide supportive care as directed by healthcare professionals.

Storage

Store Brilinta at controlled room temperature between 20 and 25 degrees Celsius (68-77 degrees Fahrenheit), away from excessive heat, moisture, and direct light. Keep tablets in their original container and store out of reach of children and pets. Do not use after the expiration date.
