Common use
Ofev (nintedanib) is a tyrosine kinase inhibitor prescribed for the treatment of idiopathic pulmonary fibrosis (IPF), systemic sclerosis-associated interstitial lung disease (SSc-ILD), and certain other chronic fibrosing interstitial lung diseases with a progressive phenotype. Nintedanib works by simultaneously blocking multiple signaling pathways involved in the fibrotic process — specifically, it inhibits receptors for platelet-derived growth factor (PDGFR), fibroblast growth factor (FGFR), and vascular endothelial growth factor (VEGFR). By targeting these three key pathways, Ofev slows the proliferation of fibroblasts, reduces excessive collagen deposition, and inhibits the formation of new blood vessels (angiogenesis) that contribute to lung tissue scarring. Clinical trials have demonstrated that nintedanib significantly slows the rate of lung function decline (measured by forced vital capacity, FVC) in IPF and other progressive fibrosing ILDs, helping preserve breathing capacity and quality of life. Dosage and direction
Ofev is taken orally as 150 mg soft capsules, twice daily (approximately 12 hours apart), with food. The capsules should be swallowed whole with water — do not chew, crush, or open them, as the contents may cause irritation. If the 150 mg dose is not well tolerated due to gastrointestinal side effects, your doctor may temporarily reduce the dose to 100 mg twice daily and subsequently re-escalate to the full dose as tolerated. Dosage adjustments may also be required for patients with mild hepatic impairment. Regular monitoring of liver function tests (ALT, AST, bilirubin) is recommended before starting treatment, monthly for the first 3 months, and periodically thereafter. Follow your healthcare provider's instructions closely and do not adjust the dose without medical guidance.
Precautions
Before starting Ofev, inform your doctor if you have liver disease or elevated liver enzymes, bleeding disorders or use anticoagulants, cardiovascular disease (including a history of heart attack or ischemic events), gastrointestinal conditions (ulcers, diverticulitis), planned surgery, or if you are pregnant, planning pregnancy, or breastfeeding. Nintedanib is teratogenic — women of childbearing potential must use effective contraception during treatment and for at least 3 months after the last dose. Monitor for signs of liver injury (jaundice, dark urine, right upper quadrant pain, unexplained fatigue), bleeding (nosebleeds, blood in stool or urine, coughing up blood), gastrointestinal perforation (severe abdominal pain), and cardiovascular events. Avoid activities with a high risk of bleeding or injury while on this medication.
Contraindications
Do not use Ofev if you are allergic to nintedanib, peanut, soy, or any component of the capsule formulation (Ofev capsules contain soy lecithin). It is contraindicated in patients with known hypersensitivity to the active substance or excipients. Ofev should not be used during pregnancy due to its teratogenic potential. Use with extreme caution in patients with severe hepatic impairment (Child-Pugh C), as nintedanib has not been studied in this population and significant liver toxicity may occur.
Possible side effect
The most common side effect of Ofev is diarrhea, affecting a large proportion of patients and typically occurring within the first 3 months of treatment. Other common side effects include nausea, vomiting, abdominal pain, decreased appetite, weight loss, and liver enzyme elevations (ALT/AST). Serious side effects requiring immediate medical attention include severe or persistent diarrhea leading to dehydration, hepatotoxicity (jaundice, liver failure), arterial thromboembolic events (heart attack, stroke), bleeding events (gastrointestinal hemorrhage, intracranial bleeding), gastrointestinal perforation (sudden severe abdominal pain), and serious allergic reactions. Seek medical care immediately if you develop severe diarrhea unresponsive to anti-diarrheal medication, signs of liver damage, chest pain, sudden neurological symptoms, or unusual bleeding.
Drug interaction
Ofev may interact with anticoagulants and antiplatelet agents (increased bleeding risk), P-glycoprotein (P-gp) inhibitors such as ketoconazole and erythromycin (which may increase nintedanib levels), P-gp inducers such as rifampicin, carbamazepine, phenytoin, and St. John's Wort (which may decrease nintedanib effectiveness), and other medications metabolized by CYP3A4. Full-dose anticoagulation therapy requires careful benefit-risk assessment when combined with nintedanib. Inform your doctor of all prescription drugs, over-the-counter products, and supplements you are taking.
Missed dose
If a dose is missed, skip it and take the next dose at the regular scheduled time. Do not take two doses at once or take additional capsules to compensate for a missed dose. Do not exceed the recommended maximum daily dose of 300 mg.
Overdose
Symptoms of nintedanib overdose may include severe gastrointestinal effects (intractable diarrhea, nausea, vomiting, abdominal pain), liver function abnormalities, or bleeding. Seek immediate medical attention if overdose is suspected. There is no specific antidote. Treatment is supportive and symptomatic with close monitoring of liver function, fluid balance, and hemodynamic status.
Storage
Store Ofev at controlled room temperature between 68°F and 77°F (20°C to 25°C), away from moisture and excessive heat. Keep capsules in their original container or blister pack until ready to use. Store securely out of the reach of children and pets.
